Over the past year we have all seen the public awakening over the need to have our personal desires declared when it comes to decisions involving life sustaining measures or to allow death by the natural course of events without medical intervention.
As part of our mission to serve the public and protect people's rights, it only seems natural that the North Carolina Advocates for Justice should offer to the public, on our website and through our members, a free "Declaration of Desire for Natural Death."**

General Information
The most important documents you need for a "living will" are the Declaration of Desire for a Natural Death and the Health Care Power of Attorney. These two documents work together to alert family and physicians to your wishes as well as to give the person of your choosing the power over your health care decisions should you become incapable of making those decisions yourself. The Advance Instruction for Mental Health Treatment and Organ/Tissue Donor Card are documents that some people might also wish to fill out; the Advance Instruction for Mental Health Treatment does require the presence of witnesses and a notary public and can be processed with the same lawyer you use for the "living will" documents.
